It all starts with the cargo and the trade lane. Experience and good judgment are needed to draw up the right specification. Insulation and heating are things needed when a cargoes melting or freezing point are above the ambient temperature in area of operation. In other instances insulation can serve to shield the cargo from direct heat. The specification must be drawn up according to the cargo NOT necessarily to what everyone else does. ‎In this case tanks were required for carriage of fuels. Here steam heating and insulation would only add to cost but would be redundant right from the start.
